• By submitting this form, I agree that Westport Veterinary Clinic will use this information to complete an Animal Health Certificate (AHC) and any errors or omissions in the information I have provided may invalidate the AHC. 
• Any invalidation of the AHC caused by errors or omissions in this form is entirely at my own risk. 
• I am aware that animals will require Tapeworm treatment between 24hrs and 120hrs before re-entering the UK. 
• I am aware that is my responsiblity to check the latest government guidelines about Pet Travel to the EU. 
• I am aware that there may be a £100 "short notice" surcharge if this form is submitted LESS than three weeks before date of travel. This will depend on how easy it will be for the vet to complete the form in time for your appointment.
• I am aware all fees must be paid in advance of my appointment.

The paperwork to complete these documents are long and complex.  The majority of the certificate is completed by the vet in advance of your appointment using the details you have provided today.
